The Admissions Process
To begin the admissions process, potential residents must go through the Golden Gate Regional Center (GGRC). The GGRC nursing team will assess the resident to determine their specific needs. If the needs are verified to align with the services provided by this home, the referral process will then commence.
Contact your loved one's GGRC Social Worker to notify them that you are looking for placement.Â

If you know that you are interested in Jerry's Care Home, indicate this in your communication.Â
The GGRC Social Worker and the GGRC Nursing Team will evaluate if your loved one is eligible for placement in Jerry's Care Home based on their individual needs. If their needs can be met in this Home, the GGRC Team will send Jerry's Care Home Team a referral packet
Once the Jerry's Care Home Team reviews the referral packet, and agrees that the Home can meet the individual's needs, we will set up a meeting to discuss any follow up questions and discuss next steps. Sometimes this may include a meet and greet, a home tour, an in-person meeting, or other assessments requested.Â
Once the assessments are done, we will coordiante the items for moving in such as supplies, care plans, staff training and other necessary steps for a smooth and safe transition. A tentative move in date will be set.Â
